GK Brothers Construction of Pequannock, N.J., was recently fined $38,025 after the death of 21-year-old worker's death in November.

OSHA said in an inspection report a laborer with GK Brothers was raised by a rough terrain forklift to an upper floor in order to retrieve a ladder. The front end fork attachment detached while lowering the man, making him to fall 20 to 30 feet, according to the report.

OSHA records showthe firm was previously cited on two other occasions for safety violations for work-related falls.
